To make a New or Renewal application for an Activity Permit (to lead or supervise on the Water), use the HQ Application Form, and the HQ Assessment Form (make arrangments to have sections 3 & 4 completed) once both forms completed, send them together with a Passport size Photograph (or email a digital photo only) to Chris Billett.
